Social Services are provided to meet the psychosocial needs of the resident. The Social Services Director works with the Director of Nursing Services and Administrator to arrange for the admission and discharge of residents. The Social Services Director acts as a liaison between the nursing home and community agencies and is available to residents and their families to help with applications and other forms required by Medicaid, Social Security, Veteran’s Administration, and other benefit programs the resident might be entitled to. In the event of discharge from the nursing home back to the community, the Social Services Director will coordinate the establishment of support services in the community. The Social Services Director participates in the development of the Resident Plan of Care working closely with Nursing, Dietary, Activities, Laundry, and Housekeeping to see that the resident’s physical, social, spiritual, and emotional needs are met.
